Privacy Policy
Geobay Exercise Rehabilitation is committed to protecting your privacy. This policy explains how we collect, use, store and disclose personal information, and how you can access it or make a complaint.
We are bound by the Australian Privacy Principles in the Privacy Act 1988 (Cth).
What we collect
We collect personal information including your name, date of birth, contact details, and emergency contact.
As a health service provider we also collect health information, which is sensitive information under the Privacy Act. This may include your injury or condition, medical history, medications, treatment and imaging reports, physical assessment findings, and your progress during rehabilitation.
Where your care relates to a workers' compensation claim, we also collect your claim number, insurer, employer, job role and physical work demands, certified work capacity, and correspondence with your case manager and treating doctors.
How we collect it
Directly from you, in person, by phone, by email, or through the referral form on this website.
We also receive information from people who refer you — insurers and case managers, employers and return-to-work coordinators, GPs and specialists, and other allied health providers. Where someone refers you, we rely on them having your consent to share your information with us.
Why we collect it
To assess your condition and deliver rehabilitation; to communicate with your treating team and referrer; to report on your progress and capacity; to administer appointments, billing and claims; and to meet our legal, professional and insurance obligations.
Who we disclose it to
With your consent, we may disclose your information to your treating doctors and other allied health providers; your insurer and case manager; your employer or return-to-work coordinator, limited to information about work capacity and suitable duties; and [ANY OTHER PARTIES].
Where your care is funded through a workers' compensation claim, reporting to your insurer, employer and treating doctor is a normal and necessary part of the service. You will be asked to sign a consent form covering this.
We may also disclose information where required or authorised by law.
Overseas storage
We use third-party services to run this website, our forms and our email, including Wix, which stores data on servers outside Australia. By providing your information you consent to it being stored overseas. We take reasonable steps to ensure these providers protect your information appropriately.
Storage and security
We store records. We take reasonable steps to protect information from misuse, loss, and unauthorised access.
We retain health records for at least seven years from your last appointment. For clients under 18, records are retained until they turn 25.
Access and correction
You can request access to the personal information we hold about you, and ask us to correct anything inaccurate. Contact us using the details below. We may need to verify your identity, and in limited circumstances permitted by law we may decline access — if so, we'll explain why.
Website
Our website may use cookies and analytics tools to understand how visitors use the site. This information is not used to identify you personally. You can disable cookies in your browser.
Information submitted through our referral form is transmitted over an encrypted connection. We ask that clinical documents and imaging not be attached to the form — contact us and we'll arrange secure transfer.
Complaints
If you're concerned about how we've handled your information, contact us first and we'll respond within 30 days. If you're not satisfied, you can complain to the Office of the Australian Information Commissioner at oaic.gov.au or 1300 363 992.
Changes
We may update this policy. The current version is always available on this page.
